School Forms and Lists

Back-to-School Forms for 2010-2011

All forms are not needed by every student and some forms have space for more than one child. Information for each form is listed below along with a link to download the forms in PDF.

* Cover Letter with Instructions.

* Emergency Treatment Form--One form needed for each student.

* Parent Pledge--For students in grades 6-12.

* School/Class Trip Authorization--One form needed for each student.

* Lower School Carpool Information--For all lower school students.

* Lower School & Key School Before- and After-School Program--Needed only if your child arrives before 8 A.M. or needs to stay after 3 P.M.

* Middle School After-School Program--Needed only if your child needs to stay after 3:45 P.M.

* Student Policy for the Ethical Use of Technology (for grades 4 - 12). Most divisions will discuss this in advisory and class meetings.

*Student Health Update Form

* Student's Physical/Immunization Report--Must be completed by a physician for all new students and annually for students in PK, kindergarten, first grade, fourth grade, seventh grade, and tenth grade.

* Authorization for Prescription Medication to be Given During School Hours. This form is to be completed by a physician.

* Authorization for Non-Prescription Medication to be Given During School Hours. This form is to be completed by the parent.
