Admissions Checklist

Phase I

1. Information packet reviewed by family.
2. Visit and tour by family scheduled through the Admissions Office. Call 828-274-0757.
3. Print out an application to mail in or fax to the Admissions Office at 828-274-0756.

After you have downloaded Acrobat Reader, click on the forms you need and print them out.

Phase II

1. Application returned with application fee.
2. Classroom visit and testing scheduled through the Admissions Office.
3. Official school transcript sent to Carolina Day School.
4. Confidential teacher recommendations sent to Admissions Office by current teacher.
5. Student Supplement submitted to Admissions Office (Grades 9-12).
6. Admissions interview scheduled through the Admissions Office (Grades 9-12).
7. Admissions Committee evaluates candidate.

Phase III

Upon acceptance to Carolina Day School, an enrollment packet will be sent with a letter of acceptance including:

• Enrollment contract
• Registration form
• Other forms depending on grade level

When your deposit and contract are received by the school, your child's place will be secured in the appropriate class.
